Live Well Program Rewards (formerly known as the Wellness Credit)
The Live Well Program offers an annual incentive that provides a health insurance premium reduction of $120 annually for Norfolk Healthcare Consortium employees enrolled in the Anthem medical plan.
All employees enrolled in the Anthem medical plan have the opportunity to earn the health insurance premium reduction by completing the healthy activities. You will have two ways to access the selection of healthy activities, either through anthem.com or the Sydney Health app.
Employees will choose activities to complete from the menu of options (such as: logging activity minutes; completing a health assessment; setting an action plan; and more) and must accumulate 100 points and complete a preventive exam to earn the Live Well Program annual incentive. The health insurance premium reduction of $120 will be applied the following year. The deadline to complete these activities is August 31st, there are no extensions to this deadline.
